  12.           The following pages provide information pertaining to each family
  13.      group  that  includes  at least one child. The pages are listed in al-
  14.      phabetical order by the husband's last name because,  in  most  cases,
  15.      that is the name used by all members of the family group.

  17.            These records follow a format that has been used by genealogists
  18.      for many years and serve as a common  research  document  that  is  in
  19.      widespread  usage.  Because  the  family  groups  are defined as those
  20.      having at least one child, and, in some cases,  the  marriage  records
  21.      may  be unavailable, the headings "husband" and "wife" should actually
  22.      read "father" and "mother". There are also some entries in  the  forms
  23.      that  do  not apply to everyone, such as date of death for individuals
  24.      that are still living.
